    What the test measures

    Vitamin B12 Active. Also called holotranscobalamin, this is the B12 bound to the protein that delivers it into cells, so it reflects the supply your body can use.

    Who should test

    Most B12 in the blood is bound to a carrier protein that cells cannot take B12 from. A standard total B12 test counts all of it, so a result can look normal while the usable fraction is low. Active B12 measures only the part on its way into cells. That makes it a more direct signal of a real shortfall, and it tends to fall earlier when supply drops.

    B12 is needed for healthy nerves, red blood cells and energy. Low levels can cause tiredness, pins and needles, poor concentration and low mood, often slowly enough that people put it down to stress. It is more common with plant-based diets, with age, after stomach surgery, and in people taking metformin or long-term antacids.

    How to prepare

    • Follow the fasting instructions for the full screen.
    • Tell your consultant about any B12 supplement or injection, and when you last took it, because recent doses raise the result.

    Reading your results

    If it comes back low

    A low active B12 suggests your cells may not be getting enough. The next step is often working out why, since diet, absorption and some medicines all play a part. Your consultant will go through it with you and suggest when to speak to your own doctor.

    If it is borderline

    Results in a grey zone are common. They are read alongside your diet, symptoms and blood count, which is part of the same screen, rather than on their own.

    If it comes back high

    High results usually reflect recent supplements. Without supplements, a raised level is less common and is worth discussing.

    Reference ranges differ between labs, and a single result is a snapshot. Your consultant reads every value against your history and the rest of the screen, not in isolation.

    How the home test works

    What is the difference between active B12 and total B12?
    Total B12 measures all the B12 in your blood, most of which is bound to a protein cells cannot use. Active B12 measures only the portion that is delivered into cells, so it is a closer measure of what your body can actually use.
    My total B12 was normal. Could my active B12 still be low?
    Yes, it can happen. A total result in the low hundreds in particular can hide a low active fraction, which is why the screen measures active B12.
    Who is most likely to be low in B12?
    People on vegan or mostly plant-based diets, older adults, people with gut conditions or past stomach surgery, and people on long-term metformin or antacids.
    Can I book a B12 test on its own?
    No. Active B12 is part of the full 122-parameter screen and comes from the same home draw as every other marker.
