Ferritin shows how much iron you have in reserve. It often drops long before anaemia shows up on a blood count.
Ferritin Levels. The protein that stores iron, so the amount in your blood reflects the size of your iron reserves.
Serum Iron, TIBC and % Transferrin Saturation. Measured on the same draw, these show how much iron is circulating right now and help explain an unusual ferritin result.
Your body uses its iron stores to keep haemoglobin up. As stores run down, haemoglobin stays normal for a while, so a standard blood count can look fine. Low ferritin is the earlier sign. It is linked with tiredness, breathlessness on the stairs, hair shedding, poor exercise recovery and restless legs.
Low iron stores are most common in women who menstruate, during and after pregnancy, in endurance athletes, in regular blood donors and in people who eat little red meat. Ferritin can also run high. Because it rises with inflammation, infection, liver strain and alcohol, a high result is read alongside the rest of the iron profile and inflammation markers.
Many lab ranges start at around 15 ng/mL, but several guidelines treat ferritin under 30 ng/mL as low iron stores. Low stores can cause symptoms before anaemia appears. Your consultant will look at why stores may be low and what to discuss with your own doctor.
High ferritin is often caused by inflammation, liver strain or alcohol rather than too much iron. If transferrin saturation is also high, it can point to iron overload, which needs follow-up with your own doctor.
