Many routine checks report TSH alone. The screen runs a full thyroid panel, TSH, FT3 and FT4, so you can see the signal and the response together.
TSH. Thyroid stimulating hormone, the signal from the pituitary gland asking the thyroid to work. It rises when the thyroid is underactive and falls when it is overactive.
Free T4 (FT4). The main hormone the thyroid produces, measured in the free form the body can use.
Free T3 (FT3). The more active thyroid hormone, mostly made from T4 in the body's tissues.
TSH is a useful screening number, but it is a signal, not the hormone itself. Free T4 and free T3 show what the gland is actually producing. Reading all three together makes it easier to tell a clear pattern from a borderline one, such as a TSH near the top of the range with free hormones that are low or normal.
The thyroid sets the pace of your metabolism. An underactive thyroid is linked with tiredness, feeling cold, weight gain, dry skin, constipation and low mood. An overactive one is linked with a racing heart, anxiety, weight loss, heat intolerance and poor sleep. Thyroid problems are more common in women and often run in families.
A high TSH with low free T4 is the common pattern of an underactive thyroid. A high TSH with normal free hormones is often called subclinical and is usually rechecked. Your consultant will explain what your pattern means and when to see your own doctor.
A low TSH with high free T4 or free T3 can suggest an overactive thyroid. Illness, some medicines and biotin can also shift results, so they are read in context.
Thyroid ranges differ between labs and change in pregnancy. A result is read against the lab's own range and your symptoms, not a single number from the internet.
