Cortisol is often called the stress hormone. It follows a strong daily rhythm, so a single blood test is a snapshot of one moment, best taken in the morning.

Cortisol rises sharply in the early morning, peaks soon after waking and falls through the day to its lowest around midnight. That rhythm is why timing matters. A morning sample is the standard, because reference ranges are built around it and results can be compared from one test to the next.
Many people want to check cortisol because they feel stressed, wired or exhausted. A blood level can be a useful part of that picture, but it is one reading on one morning. Poor sleep, a hard workout, illness, shift work and even the stress of the draw can shift it. It is most useful read alongside how you feel and your other results, such as thyroid, blood sugar and iron.
A high morning cortisol is often linked with stress, poor sleep, illness, intense training or the contraceptive pill. Persistently high levels from a medical cause are uncommon and are confirmed with specific repeat tests, not a single sample.
A low morning level can reflect timing, recent steroid medicines or, rarely, a problem with the adrenal or pituitary glands. An unexpectedly low result is a reason to speak to your own doctor.
Cortisol changes hour to hour. One reading cannot show your whole daily pattern, so your consultant reads it in context and may suggest repeating it.
