Free T3 is the thyroid hormone that does most of the work inside your cells. Most of it is made from T4 in the tissues, not in the thyroid itself.
Free T3 (FT3). Triiodothyronine that is not bound to carrier proteins. It is the most active thyroid hormone and sets the pace of metabolism in tissues such as muscle, liver and brain.
The thyroid releases mostly T4, and the body converts it into T3 as needed, largely in the liver and other tissues. Free T3 shows the result of both steps: how much the gland makes and how well it is being converted. The thyroid test page covers how TSH, FT4 and FT3 work together as a panel. FT3 responds to how the body is coping. Hard calorie restriction, heavy training, poor sleep, illness and recovery from surgery can all lower it while TSH and FT4 stay normal, as the body saves energy. That pattern is not the same as an underactive thyroid. At the other end, FT3 can rise before FT4 in some overactive thyroid conditions. This is why the screen measures FT3 rather than relying on TSH alone.
UAE labs report free T3 in pmol/L. Many adult ranges sit roughly between 3.1 and 6.8 pmol/L, but ranges vary by lab and method, so read your result against your own report.
A low FT3 with a high TSH can be part of an underactive thyroid. A low FT3 with normal TSH and FT4 is commonly linked with illness, calorie restriction, heavy training or poor sleep, and often recovers when those ease.
A high FT3 with a low TSH is commonly linked with an overactive thyroid, and FT3 is sometimes the only hormone raised. Biotin and the timing of thyroid medication can also affect it. A high result is a reason to speak to your own doctor.
Reference ranges differ between labs, and a single result is a snapshot. Your consultant reads every value against your history and the rest of the screen, not in isolation.
