ESR is one of the oldest blood tests still in daily use. It is a slow, steady signal of inflammation that complements the faster-moving CRP.
Erythrocyte Sedimentation Rate (ESR). How far red blood cells settle down a tube in one hour. Inflammation adds proteins to the blood that make red cells clump and settle faster.
CRP and full blood count. Measured on the same draw. CRP shows short-term changes, and the blood count can explain an ESR shifted by anaemia.
ESR is a general marker. A raised result suggests there is inflammation somewhere, but not where or why. It rises more slowly than CRP and can stay raised for weeks after an illness, so it is useful for spotting inflammation that has been running in the background for a while, such as in some joint or autoimmune conditions.
Many things besides inflammation shift ESR. It rises naturally with age, is higher in women, and goes up with anaemia, pregnancy and kidney disease. That is why it is never read alone. Seeing it next to CRP, haemoglobin and the white cell count from the same draw helps show whether a high number is likely to matter.
ESR is reported in millimetres per hour. Upper limits vary by lab, age and sex. A rough rule some labs use is age divided by 2 for men and age plus 10, divided by 2, for women.
A mildly raised ESR is common with recent infection, anaemia, pregnancy and older age. A clearly high or persistently raised result, especially with symptoms such as joint pain, weight loss or night sweats, is worth following up with your own doctor.
A low ESR is usually not a concern. It can be seen with a high red cell count and some inherited red cell conditions, such as sickle cell disease.
Reference ranges differ between labs, and a single result is a snapshot. Your consultant reads every value against your history and the rest of the screen, not in isolation.
