HDL helps carry cholesterol away from the arteries. A low level is one of the most common patterns on a lipid profile, and it often says as much about blood sugar as about fat.
HDL cholesterol. High-density lipoprotein collects extra cholesterol from the tissues and carries it back to the liver to be cleared. Higher levels are generally linked with lower heart risk.
Total cholesterol, LDL and triglycerides. Measured on the same draw. HDL is read with these values, and the ratio of total cholesterol to HDL is often used as a simple risk guide.
HDL is often called good cholesterol because people with higher levels tend to have less heart disease. A low HDL is linked with higher risk, even when LDL looks normal. That is why a full lipid profile is more useful than total cholesterol alone.
Low HDL often comes with raised triglycerides and extra weight around the middle. This pattern is closely linked with insulin resistance, which the screen also measures. Smoking, a mostly desk-based day, a diet high in refined carbohydrate and some inherited traits also lower HDL. Regular activity, weight loss and stopping smoking are among the things most often linked with a rise.
HDL is reported in mmol/L, mg/dL or both. Multiply mmol/L by 38.7 to get mg/dL. Many labs treat HDL under 1.0 mmol/L (40 mg/dL) in men and under 1.3 mmol/L (50 mg/dL) in women as low, though cut-offs vary by lab.
Low HDL is linked with higher heart risk, especially alongside high triglycerides or raised blood sugar. Your consultant will read it with your insulin, HbA1c and the rest of the lipid profile, and talk through what is most likely driving it.
Higher HDL is usually a good sign. Very high levels, above about 2.3 mmol/L (90 mg/dL), do not always mean extra protection and can be linked with heavy drinking or inherited traits, so they are read in context.
Reference ranges differ between labs, and a single result is a snapshot. Your consultant reads every value against your history and the rest of the screen, not in isolation.
